Discussing a DNR with a patient (Bad Example) Narrator: This is a scenario in which the staff nurse is discussing DNR options with a patient in their room. Our patient, Taylor, who is diagnosed with late, untreatable cancer, has just finished discussing a DNR with her provider. The staff nurse, Ava, was in the room during the discussion, and feels the patient does not really comprehend the meaning of a DNR order. The nurse has set some time apart to talk to Taylor to discuss her DNR order further. Staff Nurse: Hi (whoever’s name is playing the patient), I just wanted to talk to you a little bit about a DNR. Do you know what that is? A DNR means that in the event something happens such as you stop breathing and would need CPR we won’t provide life-saving measures for you. I understand you are concerned about this and have some questions but I don’t really have the time to go into detail about it now. I have other sick patients to take care of. I understand that this is a tough decision for you to make but like I said I have other things to do so if you could just give me a yes or no that’d be great. I just need something to chart ok. Patient: Hi nurse, this is a really big decision, I would like to speak with my family before I choose life saving measures or not. Can you give me some time to talk it over with them before I have to give an answer? I am really upset, I am not ready to die yet and leave my family behind. Life has gone by so fast and I am still coming to terms that this may be the end. I still have so much I want to do. Staff nurse: It really is not a big deal, I need you to give me a yes or no answer. Part of the aging process is dying. I have 3 other patients that I need to tend to, I really don’t have the time to be talking everything over with you. I need an answer to put in your chart now so that I can get on with my day and care for my other patients. If you do not give me an answer, I will just put an answer in your chart that I think will be best for you. Patient: I am not really comfortable making this decision so fast. I am struggling with the feelings that my end of life is nearing. I am hoping you can work with me and understand my emotions during this difficult time. I would like to talk about the risks vs. benefits for either decision and all of the details that are involved with a DNR. Staff Nurse: I am sorry. I should not have talked to you that way. I understand that this is a big decision. You should have time to talk with your family and with your provider about making a choice on having a DNR in place. How about I call your physician, and we set up a time to talk with you and your family about a DNR order. Patient: Thank you for your understanding. I would appreciate having that set up, in the meantime I will talk to my family as well. This has been a really tough few days for me.Q1.
