Randy Rolen Patient ID: 115037 Date of Surgery: 12/10/- – – – Admitting Physican: Leon Medina, MD, Internal Medicine Surgeon: Simon Williams, MD Pulmonry and Thoracic Services Preoperative Diagnoses: Pneumothorax, loculated, with marked respiratory distress. Postoperative Diagnosis: Pneumothorax, loculated, with marked respiratory distress. Operative Procedure: Second right chest tube insertion. Anesthesia: Lidocaine 1 %. Specimen Removed: None. Procedure done at bedside under emergent conditions. Patient was placed in the left lateral decubitus position, and the right lateral chest wall was prepped with Betadine solution and draped in the usual manner. The 10th intercostal space in the posterior axillary line was anesthetized using plain 1% lidocaine. A small nick in the skin was made, after which a Heimlich percutaneous chest tube was inserted without difficulty. This was attached appropriately to the Heimlich valve, and this was subsequently placed to suction. A previous chest tube was left in place at the 5th intercostal space on the right in the anterior axillary line. A stat portable chest xray showed improvement with full inflation of the lower lobe of the right lung that previously had remained collapsed after insertion of the upper chest tube. The chest tube was sutured in place and appropriate dressing was applied. No complications observed at the time of this dictation. ____________________ Simon Williams, MD Pulmonary and Thoracic Services SW:xx D: 12/10/- – – – T: 12/10/- – – –
